Dance Workshop waiver
I understand that Dance Workshop does not give refunds once a semester commences and all sales are final; this includes class(es) missed due to holiday, vacation, illness, weather, etc. I acknowledge that in the event of city or state required studio closures or if it is deemed necessary to close to maintain the health and safety of staff and students, Dance Workshop may offer a specified make good credit with the amount to be determined by Dance Workshop, however, refunds will not be given. I acknowledge responsibility in paying tuition at designated times, any associated costumes costs, entry fees for performances or competitions and all other communicated costs. I am aware of Dance Workshops makeup policy. Three makeup classes will given per semester and must be scheduled with management. Summer session makeups must be made up during Fall or Spring Semester (Max. of 2). I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Dance Workshop, and I voluntarily assume the risks associated with such participation. I give Dance Workshop permission to you use images and/or video of students for all promotional and marketing materials.
